Aftercare support after a self harm or suicide attempt

We will stand beside you and link you in with the supports you need

Care Connect offers aftercare support to people, 16 years and over, who are at risk of, or have previously attempted suicide or self-harm. We will walk alongside you, providing therapeutic supports and connecting you to services and support.

How do we help?

• Support individuals, their families and caregivers after a suicide attempt
• Provide a dedicated worker to help access the support needed
• Provide individual case management to work towards meeting support plan goals
• Link with other local supports to improve health and wellbeing
