Support after a suicide attempt or crisis

We will stand beside you and link you in with the supports you need

Care Connect offers aftercare support to people who are at risk of, or have previously attempted suicide. We will walk alongside you, providing support and connecting you to services and community activities.

How do we help?

  • One on one support to put in place longer-term supports and community linkages.
  • Help you to develop a safety plan to reduce the risk of future harm.
  • Link you up with other services to assist you with any challenges such as mental ill health, housing, finances, and relationships.
  • Support your closest and dearest while they support you.
  • Help you find ways to self-manage your mental and physical wellbeing.

We offer Care Connect services in the following local government areas: Central Coast, Cessnock, Dungog, Lake Macquarie, Maitland, Muswellbrook, Newcastle, Port Stephens, Singleton, Upper Hunter, and Taree. You can also use our service map to find a service near you by clicking here.

Our Care Connect offices are open from 9 am to 4 pm, Monday through Friday.

Service providers can refer people to Care Connect through our online referral form. Please see button below.

Virtual Care Connect 

Care Connect now offers a virtual option, allowing you to access follow-up support after a suicide crisis by phone or video call. 

It is delivered through secure, confidential systems, allowing you to receive support from a place where you feel safe and comfortable.
