Connecting You (HYAP) is a specialised program that supports young people (aged 12-15) who are having problems at home, or who are at risk of becoming homeless, to get connected with family, friends, work, school, and the community.
Because everyone goes through tough times and sometimes young people and families need a hand.
Referrals for Connecting You come exclusively from the Department of Communities and Justice. You can not self refer to this service.

At Social Futures, our Homeless Youth Assistance Project aims to provide support for youth homelessness in the Northern Rivers region. Whether an individual is currently homeless or at risk of becoming homeless, we will help them build connections with family, friends, work, school and the community.
By identifying an individual’s unique needs and challenges, we can offer personalised support services for homeless youth. By fostering a sense of belonging and empowerment, we can help youth develop the skills and confidence to create a brighter future.
Through community partnerships and connections with local organisations, we create a network of support that extends beyond immediate assistance. We want to address youth homelessness in Northern Rivers by giving youth a future filled with possibilities and equipping individuals with the resources and support to build fulfilling lives.
