Veterans Connect

Veterans Connect supports veterans, their families, and carers in the Central Coast. We help to improve their mental health and wellbeing by setting personalised goals, building resilience and linking them to local, health, social and community services. 

At Veterans Connect, the participant will meet their Care Navigator, who will listen to their story, and identify needs then either directly assist or connect the participant to the right services. 

If you are struggling to navigate the services and grants available, the Care Navigator can help find a way forward with support and referrals. They will act on the participants advice and provide choices. They can offer help in the following areas:   

  • connection to or information about health and specialist clinical providers 
  • connection with social and community services 
  • increase understanding of your overall health and wellbeing 
  • dealing with practicalities such finding transport to appointments.
