Connecting Home – Homelessness Adults and Youth (16+)

Support for young people, adults and families experiencing or at risk of homelessness

Connecting Home supports people who are homeless, or at risk of homelessness, to identify and source sustainable housing. Our program workers do this through informing people about their options, linking to appropriate housing providers (such as Housing New South Wales, Community Housing, as well as private rental options) and community supports and services.  

Connecting Home is government funded and is a voluntary service provided to young people (16-25), adults (of any age), and families.  Our capacity to assist people is dependent on their active involvement in agreed support activities. Services are provided across the entire Northern Rivers regions (with offices in Grafton, Lismore and Tweed Heads).

Available 24 hours, 7 days a week. 

Our program workers will:

  • Assess your needs and create an action plan with you to support your search for stable housing
  • Talk through all the options that might be suitable for you
  • Link you to housing providers, community supports and government programs that might be appropriate

For people experiencing complex challenges such as illness, family problems or disability, we can refer you to the service that is right for you. If appropriate, we support people to reconnect with their families.

Services for Homeless Families

At Social Futures, we provide homeless services for families and individuals who are homeless or at risk of homelessness. We understand the challenges families face and are dedicated to connecting families with essential support services to help them find stable housing solutions. 

With help from our program workers, we can address the needs of each family, providing tailored support to navigate the system and access the resources available. We can link families to the appropriate housing providers such as Housing NSW, Community Housing and other private rental options.
