What is Youth on Track?

Youth on Track is designed to help teens (10-17 years) who are at risk of long-term involvement in the criminal justice system. We work with young people and their families to identify triggers to offending; helping to identify issues and work through solutions; to reduce risky behaviour and to make positive life choices.

How does it work?

The scheme offers intensive one on one support to young people who have had formal contact with police. Each young person is allocated a Case Manager and each case plan is tailor made and targeted to the needs of the young person.

Youth on Track Case Managers use cognitive behaviour therapy to help young people identify unhelpful thoughts and behaviours, and learn healthier skills and habits, to reduce risky behaviour.

Case Managers work closely with the young person and their families, identifying triggers to offending, while supporting those with behavioural and mental health issues.

How does a youth get involved with Youth on Track?

Referrals to the program can be made by the police and teachers.

Do I have to be part of the program if I am referred?

No, the young person’s engagement in Youth on Track is voluntary.

Where does Youth on Track operate?

Social Futures delivers Youth on Track in Coffs Harbour and Grafton.

(Three Nations, One River)

Our beautiful artwork, Bulahbu Yabur Jurgun, Yabur Balun (Three Nations, One River) is by Bundjalung Gumbaynggirr artist Dean Loadsman.

This piece represents Youth on Track (YoT) Clarence/Coffs Coast. The central circle represents Youth on Track and the three nations – Gumbaynggirr, the fig tree; Yaegal, the river mouth and ocean; and Bundjalung, the mountains.

The three nations are all connected by the river and beaches along the coast. The sunrise represents new beginnings. The male and female figures around the outside of the circle represent the youth, with the larger figures the community Elders, leaders, mentors and youth workers, all coming together to support our youth.

The coastal carpet python can be found along the Clarence/Coffs Coast and represents the support and guidance provided by YoT for our youth and all communities working together to support our youth and provide better opportunities for the future.

The outer circle also represents the three nations, linked to the central circle by the pathways that our youth will take to get their lives back on track.

The outer symbols represent the river connecting the three nations (bottom symbol) shared learning and working together (left symbol) and the stars represent our ancestors watching over us, taking care of us, and providing guidance.
